CONNECTIVITY ASSISTANCE PROGRAM
The Paiute Indian Tribe of Utah Coronavirus Disaster Relief Connectivity Assistance Program Instructions
APPLICATIONS ARE NOW CLOSED. Please reach out to email@example.com or call 435-586-1112 if you have questions about your application.
The purpose of this application is to substantiate that federal Coronavirus Relief Funds (CRF funds) are used only for purposes permitted under the federal CARES Act. The Tribe has determined that our members are continuing to experience significant hardships as a result of the COVID-19 public health emergency. The Tribe has transitioned to virtual meetings, online program applications and continues to increase digital means of communicating with our membership. The Tribe acknowledges the need to have a current device to access these services and for our youth to support distance learning.
With this knowledge, the Tribal Council has approved the following:
- An Apple iPad for all PITU Tribal Members over the age of 18
- A laptop for PITU Tribal Members who are attending a University/College
- A Chrome book for K-12 PITU Tribal Members
- Up to $1000 for connectivity expenses including cellular and internet costs (processed via direct deposit/ACH)
All adult PITU Tribal Members must complete an individual application to be eligible to participate in this program. Program participation cannot be duplicated. If you or a member of your household has 1) already received a tablet or device, 2) claimed reimbursement for the purchase of a tablet or device, and/or 3) has been reimbursed the cost of internet or wireless connectivity, you will not be eligible for assistance under this program.
Applications will be live on December 9, 2021 and must be completed by December 17, 2021. Assistance payments will be issued on a weekly basis beginning December 15, 2021. All devices have been ordered and will be distributed as soon as they arrive.
For APPLICATION QUESTIONS, please contact the Tribal Secretary at 435-586-1112, or email firstname.lastname@example.org.
